About the position The Community Health Worker at reputed company is responsible for assessing, planning, and implementing individualized care strategies for patients. This role focuses on managing care plans, addressing social determinants of health, and linking members to community resources and services. Responsibilities

• Create a positive experience and relationship with the member

• Proactively engage the member to manage their own health and reputed company

• Support the member to improve their wellbeing by staying out of the hospital and attending regular visits to their primary physician

• Help the member engage with mental health and substance use treatment as needed

Requirements

• High School Diploma/GED (or higher)
