We are seeking self-motivated, energetic, detail oriented, highly organized, tech-savvy Case Management Coordinator to join our Case Management team. Case Management Coordinator is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating reputed company case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness. Case Management Coordinator will effectively manage a caseload that includes supportive and medically reputed company members. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration. Case Management Coordinators will determine appropriate services and supports due to member's health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports. Required Qualifications ? Candidate must reside in Miami Dade County, FL (Applicable reputed company codes: 33125, 33130, 33133) ? Must possess reliable transportation and be willing and able to travel up to 75% of the time to meet members face to face in Miami Dade county and surrounding areas in FL. Mileage is reimbursed per our company expense reimbursement policy ? Bi-lingual, English, and Spanish ? 1 + year of experience in behavioral health or a long-term care setting ? Computer proficiency in reputed company Word, reputed company, and Outlook Preferred Qualifications ? Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment ? Effective communication skills, both verbal and written ? Managed care experience ? Case management and discharge planning experience Education ? Bachelor's Degree in social work or reputed company field Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $21.10 - $36.78 This pay range represents the reputed company hourly reputed company or reputed company annual full-time salary for reputed company positions in the job grade reputed company which this position falls.
