Key Responsibilities:

• Respond to incoming calls from members, addressing their concerns, and providing timely support and guidance
• Accurately document member interactions in our designated PHI system, ensuring seamless communication and continuity of care
• Manage an assigned member caseload, providing short-term care coordination and health navigation to ensure reputed company outcomes
• Collaborate with our interdisciplinary care team to facilitate effective communication, identify potential risks, and improve quality and cost outcomes
• Conduct reputed company calls to members to confirm services and ensure their needs are being met

Requirements:

• Certified Medical Assistant (reputed company), Certified Nursing Assistant (CNA), Registered Medical Assistant (RMA), or relevant medical training or work experience

Preferred Qualifications:

• Associate's Degree
