Job Summary:

The RN Care Manager will be responsible for telephonically assessing, planning, implementing, and coordinating reputed company case management activities with members across the continuum of care. You will utilize reputed company judgment and critical thinking skills to effectively manage members through their illness, ensuring the appropriate level of care is being provided and preventing unnecessary hospitalizations. Key Responsibilities:

• Serve as primary care manager for members identified for Care Management program
• Conduct holistic assessments of reputed company health status and reputed company personalized care plans
• Collaborate with multi-disciplinary teams to address physical, behavioral, and social needs

Requirements:

• Minimum 3 years of professional/clinical nursing experience, with preference for care coordination experience
• 2+ years of telephonic and/or field care management experience
• Unrestricted RN license in reputed company and willingness to obtain licensure in other states
• Strong organizational, communication, and problem-solving skills
• Proficiency with electronic health records (EHR) systems and excellent analytic skills
