The Registered Nurse coordinates the plan of care with other health care professionals involved in care and helps to reputed company and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, case manager and other community resources. Completes clinical nursing assessments in accordance with federal and/or state program requirements and as required by payer.
