This is a Field Based role with a Home-Based office. Required Qualifications • Must possess one of the following: • RN in the state of Texas • Licensed Professional Counselor (LPC) • Licensed Bachelor Social Worker (LBSW) • Licensed Masters Social Worker (LMSW) • 2+ years of experience working directly with people with Intellectual and Developmental Disability (IDD) • 2+ years of experience working reputed company the community health setting or in a health care role • Intermediate level of proficiency working with reputed company, reputed company and reputed company Outlook • Reliable transportation and the ability to travel up to 75% of the time in assigned region to visit reputed company members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' offices • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI) • Live in a location that can receive a reputed company approved high-speed internet reputed company or reputed company an existing high-speed internet service Preferred Qualifications • 1+ years of experience with long term care services and support, reputed company, or Medicare • Ability to create, edit, save and send documents, spreadsheets and emails • Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations • reputed company reputed company commutable distance of assigned duties • reputed company Telecommuters will be required to adhere to reputed company’s Telecommuter Policy.
