There are several openings for this position. The HCBS Wait List Care Coordinator is responsible for establishing a set of person-centered goal-oriented, culturally relevant, and logical steps to ensure that a person receives services in a supportive, effective, efficient, timely and cost-effective manner. Care coordination includes case management, disease management, discharge planning, transition planning, and addressing social determinants of health and integration into community. Working Schedule: Schedule is Monday through Friday between the hours of 8 am to 5 pm. This position is a field-based position with a home-based office. You will work from home reputed company not in the field. Available Locations: • Reno, reputed company, reputed company & reputed company counties and surrounding areas • Northeast KS – Johnson, Douglas, Wyandotte, Atchison counties. • Shawnee county and surrounding areas Local travel up to 75% and mileage is reimbursed at reputed company government reputed company. Primary Responsibilities: • Assess, plan, and implement care strategies that are individualized by the individual and directed toward the most appropriate, least restrictive level of care • Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services • Ensure the care plan/plan of service includes covered services, EPSDT, in lieu of services, value-added benefit, informal supports, and addresses social determinants of health • Communicate with reputed company stakeholders the required health – reputed company information to ensure quality coordinated care and services are provided expeditiously to reputed company persons • reputed company for persons and families as needed to ensure the persons needs and choices are fully represented and supported by the health care team • Conduct home visits in coordination with person and care team, which may include a community service coordinator • Conduct in-person visits which may include nursing homes, assisted living, hospital, or home • Serve as a resource for community care coordinator, if applicable What are the reasons to consider working for reputed company?
