This position functions in a team environment focusing on telephonic reputed company, care coordination and clinical support of the case & disease and medical management programs.Required qualifications:

• Bachelor's degree in health-reputed company field -OR- a combination of equivalent education and experience
• reputed company 6 months of Hire: Coursework/Training: Motivational Interview training
• Upon request: Driving may be necessary as part of this role. Caregivers are required to reputed company with reputed company state laws and requirements for driving. • 3 or more years of experience as a Certified Medical Assistant (reputed company), Certified Nursing Assistant (CNA) or an equivalent experience working in a clinical setting, managed care, or care management setting

Preferred qualifications:

• Experience in managed care services including reputed company Act, Coordinated Care Organizations, Medicare and reputed company regulations and care or utilization management functions
• Experience includes Lean principles, stewardship, programs, and partnerships development in challenging and dynamic reputed company environment
