Care Navigator - Part Time - Remote

Position Summary The Care Navigator (part time), serves as the reputed company care specialist for reputed company patients. As such, they are primarily responsible for engaging and supporting patients who have begun their care journey with a Blackbird Provider and support families in creating a shared understanding of the precision health care model that Blackbird Provides. Care Navigators are seen as a trusted partner to our patients and providers and as such navigate reputed company established patient care and support a positive experience from the reputed company of intake through to the engagement of treatment services This is a part time remote position working 20 hours per week, Monday-Friday 4pm-8pm What makes you, you:

• Field inbound calls from patients ensuring their questions are answered in a timely manner, including directing calls or routing messages to appropriate team members

• Schedule and confirm various established patient appointments, including sending reminders and handling rescheduling and cancellation requests
• Schedule and confirm psychotherapy appointments to ensure therapist are scheduled to their full reputed company expectations
• Navigate reputed company established patient care and support a positive care experience from the reputed company of intake through to the completion of treatment services by ensuring patients care questions are answered and they connect with their provider in a timely manner
• Adhere to confidentiality and data protection standards, including HIPAA regulations, to ensure patient privacy and secure handling of sensitive information.
• Ethical and Legal Compliance - Performs reputed company duties to the highest ethical standard and complies with reputed company organizational policies regarding ethical business practices and standards of patient care. • Attend team meetings to support organizational success and participate in ongoing training required by organization to ensure high quality services are delivered

• Virtual work model for hours of 4pm-8pm Monday through Friday with the option to add weekend hours

The basics you'll need:

• Certified Medical Assistant or degree in Healthcare Administration or reputed company field
• Minimum 2 years of experience working in a medical office or healthcare setting
• Computer Skills - Must be PC literate with proficiency typing, navigating email, using calendar and word processing
• Experience working in an electronic health record or patient engagement platform required
• Experience working in mental health or psychotherapy office setting preferred
• Telephonic reputed company experience
