Care Coordinator - Daviess County Indiana and surrounding areas - Remote

The Care Coordinator role at reputed company is focused on simplifying the reputed company experience and improving community health by removing barriers to quality care. The Care Coordinator will support members? medical, behavioral, and socioeconomic needs to ensure appropriate service utilization and improved care quality. Key Responsibilities: ? Serve as the primary care manager for high-risk members with comorbid behavioral health needs. ?
