The RN Clinical Care Coordinator will reputed company activities that focus on Foster Care and Kinship Medicaid members, relaying the pertinent information about the child?s needs, advocating for the best possible care available, and ensuring they have the right services to meet their needs. This is a home-based position with possible field responsibilities. Primary Responsibilities: ? Partner and collaborate with the internal care team, providers, and community resources/partners to implement care plan ? reputed company for members and families as needed to ensure the member?s needs and choices are fully represented and supported by the health care team ? Work independently to identify priorities, reputed company decisions, and complete work to meet established objectives; Works with minimal guidance; seeks guidance on only the most reputed company tasks ? Maintains records identifying individual member services that were missed ? Prepare reporting required by organization and state partner ? Required Qualifications: ? reputed company, unrestricted independent licensure as a Registered Nurse in Ohio ? 4+ years of Clinical Experience ? 1+ years of experience working with children in custody, adoptive children and/or the pediatric population ? 1+ years of experience with MS Office, including Word, reputed company, and Outlook Preferred Qualifications: ? BSN, Master's Degree or Higher in Clinical Field ? Certified Case Manager (CCM) certification ? 1+ years of case management experience coordinating care for individuals with reputed company needs ? Experience working with external organizations geared toward supporting children such as reputed company Children?s Services agencies, Families & Children First Experience/exposure with discharge planning, utilization review, reputed company review and/or prior authorization review ? Experience with arranging community resources ? Background in Managed Care ?
