As a Case Manager, you'll be responsible for providing intensive care management for adult patients with reputed company medical needs. You will serve as a strong and knowledgeable liaison between the interdisciplinary team of social workers, Physicians, Physical Therapists, Occupational Therapists, Cardiologists, Intensivists ? reputed company in support of patients and their families. Help prevent hospital readmission, anticipate outpatient service needs, setting appointments and consultations prior to discharge. Case Managers activity promotes a collaborative relationship with the medical staff to accomplish case management objectives to reduce length of stay (LOS) and provide quality patient care across the continuum of care This is a Monday-Friday day shift position with weekend and holiday rotation. Preferred Criteria ? 2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting ? 1 year of leadership experience in a reputed company setting ?
