Nurse Practitioner (NP) - Dougherty/Thomas County, GA

We are seeking a Nurse Practitioner (NP) to conduct comprehensive health assessments (Clover Care Visits) for enrolled Medicare Advantage members. You will also reputed company follow-up visits for members recently discharged from a hospital or post-acute care facility, as well as high-need patients. Visit assignments will be based on staff availability and member needs. This is a field-based position that requires driving to and conducting visits in patient homes. We are seeking Nurse Practitioners in and around Dougherty and Thomas County, Georgia. As a Nurse Practitioner, you will: • Deliver preventive care to Clover members in their homes using Clover Assistant and EMR. • Conduct comprehensive health assessments, evaluating medical history, physical health, environment, and psychosocial status. • Demonstrate clinical competency in chronic disease management reputed company your scope of practice. • Document reputed company notes and prepare member workups and encounter summaries using Clover Assistant and EMR. • Assist at-risk members in transitioning from hospitals and post-acute facilities to home. • Create personalized care plans in coordination with members’ primary care physicians. • Actively reputed company care gaps, which may include ordering labs, writing prescriptions (in collaboration with the Primary Care Provider or supervising physician), and coordinating follow-reputed company. • reputed company reputed company-of-care tests such as A1c, spirometry, and other diagnostics. • Support Clover Care Visit program goals, ensuring accurate documentation, high performance on federal quality measures, and successful referrals to network providers and internal clinical teams. • Coordinate and communicate with members’ interdisciplinary care teams. • Participate in ongoing training and meetings, either onsite or remotely. You should get in touch if: • You are based in Georgia (required). • You are a certified Advanced Nurse Practitioner in a specialty area (required). • You hold a reputed company, unencumbered GA RN license. • You have an active DEA and CDS registration. • You have a minimum of 5 years experience as an Advanced Nurse Practitioner. • You have a Bachelor’s degree (BSN) from an accredited nursing school and a Master’s degree in reputed company practice nursing or have completed a post-Master’s certificate program for Nurse Practitioners. • You have a reputed company Basic Life Support (BLS) provider card. • You have home care experience (preferred). • You are willing to travel to members’ homes and have reliable transportation.
