OHIO | Care Manager II, RN/LSW/LISW

About the position The Care Manager II (RN/LSW) at reputed company is responsible for providing support to reputed company members through care coordination and case management services. Responsibilities • Provide support to reputed company members for care coordination and case management services. , • Engage with members, caregivers, and providers to assess and plan individual member goals. , • Complete comprehensive person-centered assessments of physical, psycho-social, and environmental health. , • Identify problems/barriers for care coordination and implement appropriate interventions. , • Coordinate physical, behavioral health, and social services for members. , • Manage medications, including regular reconciliation and adherence support. , • Create and implement a plan of care to assist members in achieving health goals. , • Identify short- and long-term goals with associated time frames for completion. , • Document reputed company towards meeting goals and resolving issues. , • Schedule follow-up calls and reputed company appropriate referrals.
