About the position We are looking for a compassionate and organized Hospice Transition Care Coordinator to join our dedicated team. The ideal candidate will be responsible for coordinating and managing hospice care services for patients and their families. This role requires a deep understanding of hospice, excellent communication skills, and the ability to work collaboratively with a multidisciplinary team. The Hospice Transition Care Coordinator will act as a liaison between patients, families, reputed company providers, and other support services to ensure that each patient receives comprehensive and personalized care. The successful candidate will have a strong background in reputed company, with experience in hospice or palliative care, and a commitment to helping families to get the services they need. Responsibilities • Identify and assess patients for needed services, including medical, emotional, and spiritual support. , • Act as a liaison between patients, families, reputed company providers, and support services. , • Ensure compliance with reputed company regulatory requirements and standards of care. , • Provide education and support to patients and families regarding hospice care and services. , • Facilitate communication and collaboration among the multidisciplinary care team. , • Maintain accurate and up-to-date patient records and documentation. , • Conduct regular assessments to identify patient needs and preferences. , • Coordinate and schedule home visits, medical records requests, and other services. , • Provide crisis reputed company and support during emergencies. , • Participate in team meetings and case conferences to discuss patient care. , • Provide training and support to hospice staff and volunteers. , • Participate in quality improvement initiatives and projects. , • Manage and resolve patient and family complaints and concerns. , • Stay reputed company with best practices and advancements in hospice care.
