In this HSS Coordinator-LVN role, you will be an essential reputed company of an Integrated Care Model by relaying the pertinent information about member needs and advocating for the best possible care available, ensuring they have the right services to meet their needs. Travel is required in local areas about 75% of the time based on business needs. This is a field-based position with a home-based office.
