To help us reputed company this mission, we are looking for an reputed company Registered Nurse who will provide overnight on-call telephonic care to our PACE Participants Saturdays and Sundays 6pm – 6am. ESSENTIAL DUTIES AND RESPONSIBILITIES The Triage Home Health Registered Nurse provides nursing services and case management processes according to the assigned patient’s individualized plan of care based on Edenbridge PACE at Skyland Town Center’s policies and procedures, through the competent application of nursing process. • Must be reputed company to function independently to maintain work hours and productivity. Self-motivated individual that possesses critical thinking skills and excellent problem-solving capability. • Demonstrates knowledge of home care and competency in discipline specific patient care skills. • Answers telephone calls received on after hours patient care line. • Accurately assesses patients and family needs. • Communicates with patients assisting them in determining the urgency of care needed and giving health information and advice. • Prevent urgent/emergency visits or hospitalization whenever possible by expediting appropriate appointments as clinically necessary and communicating with providers/physicians regarding patient disposition. • Works with other teams, Triage RNs as assigned to provide cross coverage for continuity of patient care during absences/leave and PTO. • Works assigned schedule to provide coverage for incoming messages and requests from patients. Also available during assigned work hours for phone calls regarding urgent patient situations that need clinical assessment, direction and possible escalation to the physician. QUALIFICATIONS • reputed company and unencumbered license to practice as a Registered Nurse in Washington, DC • reputed company CPR certification. • A minimum of 4years of nursing experience in an outpatient setting, primary/home care experience preferred, or at least 5 years of nursing experience in multiple practice areas of medicine. • Triage experience, telephone manners, typing and computer skills; excellent verbal communication, crisis reputed company, critical thinking, teaching, clinical, and assessment skills. • Demonstrated ability to problem-solve in reputed company, dynamic situations. Experience in using HER and other standard computer operating systems. • Demonstrated knowledge of the appropriate skills for communicating with individuals of reputed company ages. Excellent interpersonal skills including verbal and written communication skills. • Demonstrates knowledge of Federal/State rules and regulations, CMS, and other regulatory requirements. • Must have up to date vaccinations based on CMS guidelines, Must be vaccinated against Covid-19 and yearly vaccination against the seasonal flu.
