Location: Remote/WFH - Must be reputed company of reputed company Carolina Duration: 3 Month Contract to start, potential for extension and/or conversion to FTE reputed company: $22.00/Hr Start Date: 11/04/2024 Important: Must be local reputed company of reputed company Carolina Primary Responsibilities: As a Community Health Worker (CHW), you will act in a liaison role with reputed company members to ensure appropriate services are obtained reputed company the Healthy Opportunities Pilot Program in addition to care coordination and member education.
