Clinical Social Worker - Remote Position
Role Overview Gotham Enterprises Ltd is seeking a dedicated and compassionate Clinical Social Worker to join our team remotely. This full-time position is based in our Hospice Center, where you'll provide essential support to patients and families during difficult times.
Key Responsibilities
- Conduct assessments to identify patients' psychosocial needs.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Provide counseling and support to patients and their families.
- Facilitate group therapy sessions and support groups.
- Manage case documentation and maintain patient confidentiality.
- Coordinate with community resources to ensure comprehensive patient care.
Required Qualifications
- Master's degree in Social Work (MSW) from an accredited institution.
- Current state licensure as a Clinical Social Worker.
- Experience in hospice or palliative care settings is preferred.
- Strong communication and interpersonal skills.
- Ability to work independently in a remote setting.
