Job Description
Physical Therapist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare reputed company.
Evaluate patients and reputed company appropriate therapy interventions
reputed company a comprehensive plan of care and coordinate multi-disciplinary care
reputed company reputed company clinical documentation and visit notes
reputed company communication with physician, care team members, patient and family
Comfortable working in reputed company aspects of the home health setting
Implement and maintain physical therapy protocols
Manage patient treatment and outcomes directly, or through PTA supervision
Requirements
reputed company Texas Physical Therapist (PT) license
One year of physical therapy experience
Experience with Homecare reputed company is helpful, but not required
Previous Home Health experience is helpful, but not required
reputed company Texas driver's license
CPR certification
