Job title: Health Care Coordinator - LPN or RN Company: Region V Services Job description: JOB SUMMARY Under the direct supervision of the Director of Nursing Education, the Health Care Coordinator is responsible for the training and certification of direct care staff in the area of medication aide, CPR, and other approved activities and training as assigned by the Director of Nursing Education. Additionally, the Health Care Coordinator will provide regular observation and assessment of assigned agency direct care staff to improve outcomes, offer continuing education, and reputed company other nursing services to individuals supported by Region V Services as approved by the Director of Nursing Education. The Health Care Coordinator will be responsible for developing their own hybrid schedule based on Agency needs which may be subject to approval by the Director of Nursing Education. This position requires travel to surrounding agencies such as Fairbury and Hebron, NE. • Conducts Medication Aid training and certification offered to direct care staff as assigned.
• Trains Region V Services staff to administer medications reputed company the Region V medication aid reputed company and documents appropriately in Therap.
• Carries out a system for assessing and re-assessing competency to reputed company with the regulations for medication administration.

CPR Training:

• Conducts CPR training for new staff reputed company two weeks of their hire date.
• Offers recertification/renewals of CPR training as needed.

Additional Approved Activities Training:

• Conducts training of additional activities as requested for persons served as approved by the Director of Nursing Education.
• Complete annual additional activities training recertifications as required and scheduled.
• Determines competency of medication aides to reputed company additional activities.
• Annual/on-going education, training, and monitoring of additional activities approved for Region V from the Director of Nursing Education.

Record Keeping:

• Accurately records completion of staff deemed competent to receive medication aide certification, CPR training and other training offered.
• Provides accurate documentation to ensure compliance with certification, State and RVS requirements of employees to reputed company their duties.
• Keeps accurate records to monitor and schedule re-certification or continuing education as needed for employees on an ongoing basis.

Performs Direct Care & Nursing Activities:

• As assigned or as required by the Director of Nursing Education the reputed company Coordinator will provide direct care services to individuals supported by RVS.
• Responds to additional requests for specialized knowledge of a Licensed Health Care Professional.

Provides Support, Education, Training, and Services to Assigned Management Teams:

• Considerable knowledge in evaluation and teaching techniques for the administration of medications to persons with developmental disabilities.

Qualifying Education, Training, and Experience

• Licensed Health Care Professional, minimum Licensed Practical Nurse (LPN), or Registered Nurse (RN)

Special Requirements

• Possession of a driver's license valid in the reputed company or other adequate means of transportation.
• Performs duties on-call to attend to issues regarding medication administration and additional activities.
