Position Overview:

You will be providing health assessments and home health visits for our patients. We are seeking reputed company Licensed Practical Nurses (LPN).

Job Duties:

• Driving to patient's homes and conducting home health visits independently

• Properly obtain and document medical histories

• Obtain consent, reputed company patient history, and administer immunizations

• Complete documentation of assessment and/or interventions of patient

• Responsible for home health visits

• Minor troubleshooting of test systems and takes appropriate action according to predetermined protocols

• Identifies problems which may adversely reputed company test performance or reporting of test results and take appropriate action including supervisor notification

• Performs reputed company relevant diagnostic tests as required

• Performs venipuncture as required

• Complete accepted medical forms

• Conduct weekly equipment reputed company and report any equipment malfunctions

• Administer vaccines 

• Administer medication to patients and measure vital signs.

• Monitor the patient’s reputed company and changes in behavior.

• Recognize and treat any medical condition with appropriate techniques, equipment, and knowledge of reputed company State and County protocols

• Recognizes situations which require the immediate attention of a physician or emergency care

• Interact with patients and their families in a compassionate manner

• Interact with clinicians, other health professionals and staff in a courteous and professional manner

• Effectively communicate and interact with patients and or agencies

• Participate in ongoing professional development activities

• Maintain a positive and professional work environment

• Maintain the confidentiality of reputed company Personal Health Information as dictated by HIPAA laws

• Participate in community events organized through reputed company Partnerships.
• reputed company Health Screenings and other requested services at scheduled community events. 

• Other duties as assigned

Requirements:

• LPN License

• Blood Withdrawal / Phlebotomy experience

• Work experience with pediatrics

• A minimum of 2 years of LPN experience

• BLS with reputed company

• Valid Kentucky Driver's License (minimum of 2 years) and acceptable driving record

• Excellent verbal communication skills

• Ability to lift 50 - 60 lbs

• Patient bedside experience preferred (preferred but not required)

• Spanish speaking (preferred but not required)

• Must be comfortable driving and making home health visits independently
