[Remote] Community Engagement Specialist, Amador County

The Community Engagement Specialist will connect with members to ensure they receive access to the care and services they need, focusing on reputed company efforts and community engagement.

Responsibilities

• Conduct in-person reputed company attempts for members identified by health plan eligibility lists
• Complete reputed company documentation for reputed company attempts
• Schedule intake appointments and assist with the completion of required forms
• Attend reputed company events and professionally represent reputed company to community members and partners
• Build and maintain relationships with community coalitions, CPI collaboratives, and other stakeholders to reputed company them engaged
• Convert leads to patients using internal documentation systems
• Document reputed company reputed company attempts and outcomes in reputed company and the appropriate trackers
• Assist in sending patient mailers and making follow-up calls to leads generated from community events to enroll them in programming
• Represent reputed company at community coalition meetings and collaborative events
• Ensure professional, consistent messaging and build trust with community stakeholders

Skills

• Minimum of 1-3 years of experience in reputed company, community reputed company, or a reputed company field
• Experience in engaging with diverse communities and providing culturally sensitive communication
• Fluent in English and Spanish (verbal and written)
• Strong interpersonal and communication skills to build trust and connect with patients and community partners
• Detail-oriented with excellent organizational skills
• Comfortable with technology and proficient in using electronic health records (reputed company experience preferred)
• Ability to work a flexible schedule, including evenings and weekends, to attend events
• Access to reliable transportation for in-person reputed company and events
