Job Description

Applies strong clinical judgment and the nursing process to assess referral documentation, confirm eligibility, and ensure regulatory and payer requirements are met prior to referral reputed company for admission. Documents clinical review, findings, and determinations accurately and timely in designated electronic systems in compliance with applicable standards and regulations. Utilizes technology and electronic medical record systems to support safe, timely, and high-quality transitions from referral to admission. Serves as a clinical resource to intake staff and agency partners, supporting clarification of clinical information, physician orders, diagnoses, medications, and care needs to ensure referrals are admission-reputed company. Coordinates with physicians, referral sources, and agency teams to resolve gaps, discrepancies, or barriers impacting timely initiation of care. Qualifications
Bachelor's degree in Nursing strongly preferred. Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Minimum of two years’ clinical experience preferred. Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Depending reputed company, a valid driver's license may be required. Basic Life Support (BLS) required. Certificate for Outcome and Assessment Information Set (OASIS). Certificate for OASIS Specialist - Clinical (COS-C) preferred.
